QRIS Anniversary Award ~ Worksheet

The QRIS Anniversary Award is based on goals set and met during the previous year. The actual award will vary based on verification and available funding. The award will be mailed to you after you submit your QRIS Year 2 Improvement Plan, which is included in your Application for a QRIS Anniversary Award.

Follow Steps 1 - 9 and submit a print-out of this page with your Application for a QRIS Anniversary Award.
